For non-acute situations we build a complete life infrastructure document. For acute situations we stabilize immediately and build the full infrastructure in parallel. We work with the individual when they retain decision making capacity and with the designated power of attorney and healthcare proxy as authority transitions. In many cases these are different people. Silver Continuity keeps them aligned.

Why we built this
Silver Continuity was founded by Rachel Barr from personal experience navigating the complexity of aging, cognitive decline, and estate administration for her own family.
My father had his affairs in order. The trusts and his estate were in place. The paper trail was there. The accountants were solid. After his car accident, it still took my family months to coordinate everything while we were raising children and holding high stakes careers. The burden was profound.
My single aunt is high net worth, has early stage dementia, lives in a premier continuing care facility with additional around the clock care, financial resources without limitation, a concierge physician, and a family that shows up. Every box is checked. And yet in the week she had her stroke I spent more than 25 hours managing her situation from out of town. None of it touched her direct care or her medical decisions. All of it was the coordination layer around it. Family. Staff. Finances. Insurance. Logistics. Months later the ongoing coordination demand remains higher than it was before.
